How Do You Create A Review Template?

The Employee's Performance Review starts with the creation of the review template. 

  • If you are given permissions to Create the Performance Review

  • Click the gear icon on your header. Navigate to the Employees tab and then go to the Reviews section. Here you will see all your review templates in your account(if any).
  • To create a new review template click on the + New Review Template  button.

  • Create a name for your template.
  • Checking the Kudos & Notes, Goals, Skills and Shared Feedback, boxes will pull them from Employee Files and brought into the review cycle.
  • When creating a review template, use the Questionnaire form builder to add desired questions to the review process. To create a new question, click the +Question link.

  • For each question, set the following:
  • Question - this is your question text
  • Description - this is a description of the question.
  • Visibility
    • Employees will see question text but will see the Manager responses after the Manager has added talking points (if any) and clicked the "Save Review".
    • Manager Only: selecting this visibility setting will make this question available only to the Manager to provide a comment during the first phase of the review process.
    • Employee Only: selecting this visibility setting will make this question available only to the Employee to provide a comment during the first phase of the review process.
      • Employees will see question text and their own responses throughout the entire review process.
      • Manager will see question text and Employee's responses throughout the entire review process
    • Employee & Manager: selecting this visibility setting will make this question available to both Employee and the Manager to provide a comment during the first phase of the review process.
      • Employees will see question text and their responses when they click on the Finish button. They will see the Manager's comments after the Manager has clicked on "Save Review"
      • Managers will see all Employee's comments throughout the entire review process.
  • Stars is a metric that is used by managers to rate employee performance, when managers are answering the question, you have the option to Enable or Disable the star rating, the Rating Comment gives you an option to create a comment regarding how you want your managers to use the star rating.
  • Enabled - when this option is enabled, star metric is visible to the Manager.
  • Disabled - when this option is disabled, stars are not visible to any users to select.
  • Click Save Question to save the question. 

 

  • Once you have added all the questions you want to include, click Save Review Template to save your review template.
